When I was first worked with a psychiatrist to try medications for ADHD and SAD, I wanted a tracker to note how the various medications affected me. Some meds helped me complete tasks, but then made me anxious. Or, I would notice a med helping me, but then wear off after a few days.
A Google search yielded many trackers to record if I took a medication, but very little in the way of tracking how effective the medications are.
So, I created this chart to help monitor the benefits and side effects of various medications. Having good notes helped me communicate what was working and advocate for myself.
You can list multiple medications that are tried concurrently, or in sequence; and you can adjust the dosages as you titrate up or down. You get to define which symptoms you want to track, and also note any relevant side effects.
